Sesamoiditis: El dedo gordo y sus dolores 4/5

Sesamoiditis
Hay dos huesos debajo de "La Bola del Pie", estos se llaman los Sesamoides.  Uno es el sesamoide tibial y el otro el sesamoide fibular. Estos tendones estan dentro de los tendones flexores del dedo  gordo.  


Estos dos huesos sirven como poleas para los tendones de la propulsión del pie.  Los sesamoideos son importantes a la gran zona de la puntera, ya que absorben la presión de soporte de peso y reducen la fricción en la cabeza del metatarsiano.
Cuando estos huesos se irritan generan un gran dolor.  Estos huesos son irritados luego de un accidente, una torcedura en el dedo o el uso constante de zapatos sin soporte en el área. El dolor es situado en la parte inferior del dedo gordo del pie.


El Tratamiento conservativo
·                Detener la actividad que causa el dolor.

·                Tome antiinflamatorios para aliviar el dolor.

·                Aplique hielo envuelto con una toalla en el área

·                Use zapatos con suela suave y tacón bajo.

·                Use cinta adhesiva para mantener hacia abajo el dedo gordo

·                Inyección esteroide

·                Uso de un CAM boot

Tratamiento quirurgico si el tratamiento conservativo no funciona
 Cualquier condición del hueso sesamoideo crónica que no responde al tratamiento conservador puede requerir cirugía.

·                Remoción de parte o todo de uno de los huesos sesamoides.  Cuando un solo sesamoide es eliminado, el otro hueso sesamoide puede todavía proporcionar un punto de fulcro para los flexores del dedo del pie.

·                Raspado del sesamoide si el paciente tiene un callo debajo de un sesamoide hipertrófico

·                Reparación y fijación de la fracture, esto se hace con tornillos pequeños y alambre metálico

Surgery is indicated in nearly every instance and will produce a corrected or improved result. The type of surgery--whether reefing, lengthening, or transposition of tendons, implant arthroplasty, or arthrodesis--is tailored to the underlying cause.  Management Dorsal and plantar-shaped incisions around the toe and extensor digitorum longus tenotomy and circumferential MTP joint release.

Shaq foot surgery



In 2002, Shaquille O'Neal was suffering from great toe arthritis.  In September 2002, Shaq received a 1st MPJ fusion. The goal of this surgery was to immobilize the joint to prevent pain and keep the basketball player functional.  

The procedure took only 45 minutes, and it was performed on an outpatient basis. A six-week rehabilitation program had O'Neal ready to play ball a week before the regular season began. The typical patient who receives this procedure can return to work in 3 weeks.

Ingrown toenails are a very common problem that podiatrist see everyday.  People get ingrown toenails for different reason: improper nail clipping, genetics, infections or trauma.  
Some tips on this issue:
  1. Clip your toenails square, avoid clipping them round or too close on the corners
  2. If you get the ingrown toenail soak your feet twice a day on Epsom salt and keep a band aid with antibiotic ointment, this will soften the nail and decrease the symptoms
  3. If you are diabetic, be careful, infections symptoms will not be felt, so inspection is very important, infection should be treated promptly
  4. If the ingrown is not getting better call your local podiatrist or primary care doctor for a prescription of antibiotic while you wait for the appointment 
Ingrown nail procedures are done by podiatrist.  The incurvated nail is excised and in many case a chemical can be added to prevent the nail corner to grow again.  Like Mark Wahlberg the day of the procedure you will have a bulky dressing then a band aid will be enough.

Diabetes is on the rise: an estimated 1.3 million adult New Yorkers (almost one in eight) now have diabetes.  Halle Berry, Drew Carey and Larry King are among the celebrities who live with diabetes.  This disease has not stopped their lives. However, Diabetes can cause serious complications like nerve damage, blindness and foot amputations. 

To prevent these problems make sure to have tight control of your diabetes, monitor your feet daily and visit a podiatrist regularly for foot exams.

Leg and ankle swelling is one of the most common complains pregnant patient present to my office.  During pregnancy hormone changes make the body retains more water and the growing uterus also press again the leg vein making it difficult to push blood from the legs to the heart.
Some tips for you and Kim Kardashian next pregnancy: 
  1. Avoid standing for long periods, and don't cross your legs
  2.  sleep on your left side
  3. Drink plenty of fluids ( ~10 cups)
  4. Take daily walks, ride a stationary bike or swim laps in a pool.
  5. apply cold-water compresses to swollen areas.
Visit your local podiatrist he/she can provide you with compression stocking.  This devices will help pump blood up your leg, decrease ankle pain, avoid any serious complications, and give you more stability.

Oprah's bunions

Bunions are a common deformity of feet.  People get bunions for many reasons but genetics is the most common.  Symptoms vary from pain on the joint to pain at the bony prominence.  The bunion deformities can occur on the inside of the foot (regular bunion) and/or outside of the foot (Tailors bunion
Oprah Winfrey has both, a regular bunion and a Tailors bunion, in this case this deformity is call a Splay foot.
Conservative treatments include padding and wider shoes.  Surgical treatments are more curative of the deformity.  Bunion surgery or bunionectomy is one of the most common procedure done by podiatrist.

Plantar fasciitis in the NBA


The Chicago Bulls player P.J. Brown was recently sidelined with a right foot plantar fascia strain. The repetitive nature of activities such as basketball —running, jumping, and other explosive movements that cause increased foot demands—are especially hard on the Plantar Fascia ligament and can cause an acute plantar fasciitis. The majority of forefoot surgeries are perform on outpatient bases in a surgical center.  

Forefoot surgery include 
  • bunionectomy, 
  • hammertoe toe correction, 
  • tailors bunion correction
  •  excision of soft tissue masses



For any Forefoot surgery expect:

·         1st-3rd week:  limited and protecteAd WB with a surgical shoe, (coach-bathroom-bed), Rest, elevate, ice (20min in, 20min out), anti-inflammatory

·         4th week: Begin ROM exercises

·         3rd-6th week: active WB, transfer to tennis shoe progressively

·         4th week : return to sitting job,

·         5-6th week: return to walking job
