Sesamoiditis: El dedo gordo y sus dolores 4/5

Sesamoiditis
Hay dos huesos debajo de "La Bola del Pie", estos se llaman los Sesamoides.  Uno es el sesamoide tibial y el otro el sesamoide fibular. Estos tendones estan dentro de los tendones flexores del dedo  gordo.  


Estos dos huesos sirven como poleas para los tendones de la propulsión del pie.  Los sesamoideos son importantes a la gran zona de la puntera, ya que absorben la presión de soporte de peso y reducen la fricción en la cabeza del metatarsiano.
Cuando estos huesos se irritan generan un gran dolor.  Estos huesos son irritados luego de un accidente, una torcedura en el dedo o el uso constante de zapatos sin soporte en el área. El dolor es situado en la parte inferior del dedo gordo del pie.


El Tratamiento conservativo
·                Detener la actividad que causa el dolor.

·                Tome antiinflamatorios para aliviar el dolor.

·                Aplique hielo envuelto con una toalla en el área

·                Use zapatos con suela suave y tacón bajo.

·                Use cinta adhesiva para mantener hacia abajo el dedo gordo

·                Inyección esteroide

·                Uso de un CAM boot

Tratamiento quirurgico si el tratamiento conservativo no funciona
 Cualquier condición del hueso sesamoideo crónica que no responde al tratamiento conservador puede requerir cirugía.

·                Remoción de parte o todo de uno de los huesos sesamoides.  Cuando un solo sesamoide es eliminado, el otro hueso sesamoide puede todavía proporcionar un punto de fulcro para los flexores del dedo del pie.

·                Raspado del sesamoide si el paciente tiene un callo debajo de un sesamoide hipertrófico

·                Reparación y fijación de la fracture, esto se hace con tornillos pequeños y alambre metálico

Nice shoes, nasty toes

A condition that usually affects the big toe or the fifth toe, which is characterised by loss of function of the interossei and lumbricale muscles resulting in overpowering of the flexor tendon with resultant overriding toe and depressed metatarsal heads, both prone to ulceration, soft tissue tears, digital dislocation or painful calluses.


Surgery is indicated in nearly every instance and will produce a corrected or improved result. Painful corn!!!!


Calluses or Corns 
are an area of skin that thickens after exposure to repetitive frictional forces in order to protect the skin.  Calluses are painful. 

Factors within the body that may lead to the formation of corns and calluses. 
  • Bony prominences or structural deformities, such as hammertoes and bunions
  • Faulty foot function such as overpronation or oversupination
  • Damaged sweat glands, scars, warts(plantar verruca)
If home remedies fail to eliminate the corns and calluses and they continue to be painful or bothersome, consult your doctor. Anyone with diabetes, loss of sensation or poor circulation should seek medical attention earlier because of a higher risk for infection.

Podiatrists will take X-rays of your feet to look at underlying bony structures that may be the cause of the corns and calluses. 

Periodic shaving or cutting off the hardened area on the skin (debridement) by surgical means or with medicines that break up hardened areas of skin such as 40% urea (keratolytic agents)

Why socks?
The foot is among the heaviest producers of sweat in the body, the average foot has 250,000 sweat glands.   The foot can produce over 0.25 US pints (0.12 l) of perspiration per day. Socks help to absorb this sweat and draw it to areas where air can evaporate the perspiration.  Socks will avoiding a variety of foot problems including blisters, athlete's foot and a whole host of others.

In cold environments, socks decrease the risk of frostbite.  Footwraps, pieces of cloth that are worn wrapped around the feet, were worn with boots before socks became widely available.  In the 8th century BC, the Ancient Greeks wore socks from matted animal hair for warmth.  

Socks can be created from a wide variety of materials. Some of these materials are cotton, wool, nylon, acrylic, polyester, olefins, (such as polypropylene), or spandex.  Most sports will require some sort of sock, usually a tube sock to protect one's legs from being scraped while participating in sport activities.

Swiss researchers found that when we are on the cusp of falling asleep, our body redirects blood flow to our hands and feet. When these extremities are warm, their blood vessels can dilate and allow for greater blood flow, which of course aids in the redistribution of blood and helps you get to sleep. So if you're having trouble nodding off, the solution could be as simple as throwing on a warm pair of socks.

Shoe Wear

 


SHOES!!!!!



 The purpose of  shoes is to protect and cushion the feet.  Improperly fitted shoes  cause of many foot deformities like bunions, hammer toes, ingrown nails and corns. Foot size changes with age, is very important to have feet measured every time you buy shoes.

A proper podiatric examination has to includes the evaluation of the shoes.  Wear patterns are generally predictable, with the normal wear pattern on the outsole slightly to the inside at the toe and outside at the heel. Abnormalities in the wear pattern indicate problems with alignment and gait. Extra eyelets, where the laces criss-cross to the top of the shoe, aiming for a snug but comfortable fit.

Although proper fitting of shoes always should be fit to the weight-bearing foot at the end of the day, when feet are at their largest. The upper should not wrinkle with flexion, and the foot should not bulge over the welt. The end of the longest toe of the larger foot should be within 3/8² to 1/2² of the end of the toe box. The shoe should provide a relatively snug grip at the counter above the heel.  Above all else, shoes should be comfortable from the moment they are tried on.

Brook Lopez foot injury


Brooklyn Nets center Brook Lopez underwent surgery Jan 4 to repair a 5th metatarsal fracture. The majority of the metatarsal bones heal with nonsurgical treatment; however some will requires more extensive immobilization and surgical fixation. Swelling and pain on weight bearing are common symptoms. Stress fractures usually occur after a sudden increase in activity, such as a new training regimen, change in running surface, or even prolonged walking.  

Podiatrist can X-ray your injured foot and diagnosis of this injury. A stress fracture of a metatarsal may not show up on radiographs for 2 to 3 weeks. If this problem is not addressed in time or not properly treated a malunion and nonunion are possible. Malunion with painful plantar callosities under the metatarsal heads or transfer lesions under the neighboring metatarsal heads may occur if there is displacement or shortening of the metatarsal.

Treatment includes the use of a short leg cast, fracture brace, or wooden-soled shoe. Weight bearing is permitted in most kinds of injuries but there are exceptions. Tenderness at the fracture site will diminish as the fracture heals. A fracture of the first metatarsal and some 5th metatarsal base may require surgery. Multiple metatarsal fractures and displaced fractures may require either closed or surgical reduction.

Nail colors

Nail can turn different colors for different local or systemic problems.  Nails can change color because of infections that involve the nail itself, the bed under the nail, or the skin around the nail. Most nail infections are fungal infections, but bacterial and viral infections can occur. Bacterial infections may occur in the cuticle or nail folds (paronychia).
 

Green nails- are a sign of bacterial infection

Red streaks - in your nail bed are a warning of a heart valve infection

El tobillo de Carmelo Antony



 Todos los neoyorquinos recordamos que hace casi un año, nuestro Knicks StarCarmelo Anthony estuvo ausente de varios juegos debido a una torcedura del tobillo izquierdoEste año, Antony estuvo ausente por varios juegos por la misma condición del tobillo izquierdo. Estamos contentos que estuvo de regreso para El Juego de Estrella 2014


En una entrevista Antony describió como leve el dolor inicial y que inicialmente el tobillo fue tratado con cinta adhesiva. Al día siguiente la inflamación era significativa y más dolorosa. 

Las torceduras de tobillo tienden a ser muy recurrente. Durante una torcedura de tobillo el ligamento que sujeta el tobillo se estiran. Esto afecta a los nervios dentro del ligamento que le dicen al cerebro la posición del tobillo. Una vez que estos nervios no funcionan correctamente los pacientes desarrollan torceduras recurrentes del tobillo. 

Hay diferentes maneras de tratar conservativamente las torceduras de tobillo. El dolor y la inflamación son tratados con terapia oral, reposo, hielo y compresión. 

Para prevenir la recurrencia de las torceduras de tobillo recomendamos terapia física y un refuerzo del tobillo. 

El tratamiento quirúrgico es una alternativa al tratamiento conservadora cuando este falla o en pacientes tiene alta exigencia física. La cirugía consiste en la reparación del ligamento lesionado y en algunos casos refuerzan el ligamento con tendones adyacentes. Esta cirugía es realizada comúnmente por podiatras en Estados Unidos. 

El tiempo de recuperación necesario es de 1 ó 2 semanas, durante ese periodo no debesoportar un peso, un caminar durante 2-3 semanas. La fisioterapia es esencial para la adecuada recuperación quirúrgica.
